Dementia Care Nurse
At Trelleborg, we work together for a growing successful municipality where it is good to live. Our 61 villages and 21 city districts are creative meeting places that buzz with life. Here we want things, talk about them, and come as we are. This means we welcome others in the same generous and curious way. Welcome to be part of the journey! The Social Services Department's mission is to work with social services and provide support so that everyone in the municipality feels secure and has equal opportunities in life. We want everyone to be able to be active and involved in society. With a focus on respect for the individual's right to make decisions about their own life, we are responsible for three important areas: care and support, support for people with disabilities, and assistance to individuals and families. Our goal is for everyone to receive the support and care they need. Since everything Trelleborg Municipality does is based on our core values of openness, respect, and responsibility, it is important and crucial that these values are just as self-evident for you as they are for us in our work. The Workplace As an employee with us, you become part of the Nursing Unit in Trelleborg Municipality, a competent and supportive organization where nurses, specialist functions, and other professionals work together to create safe and person-centered care. Here we work closely with each other and have a climate characterized by collaboration, collegial support, and a shared commitment to developing the municipality's care and support services. Your physical base is at the Nursing Unit in Trelleborg. Here you work closely with, for example, MAS/MAR, family support, and benefits officers — important partners in daily work. The role involves both on-site work, home visits within the municipality, and contact via telephone and digital tools, which provides good variety and flexibility in your work methods. We are in a development phase where both the national dementia strategy, new social services legislation, and the transition to good and accessible care create opportunities to shape the future of dementia work. With us, you make a concrete difference for people with cognitive illness and their loved ones. We offer you varied and independent work where you get the opportunity to use your specialist competence in practice. You receive an individually adapted introduction that gives you a secure start in the role, as well as the opportunity for municipality-wide benefits. Work Tasks In this role, you function as a central knowledge resource in the municipality's dementia work. You provide support and guidance to people with cognitive illness, their loved ones, and to staff. The work is characterized by a person-centered approach and is based on creating good collaboration between all involved parties. You work both independently and together with other professionals. The role includes mentoring and training staff and, when necessary, participating in patient-related meetings. You work from a holistic perspective where coordination in the care chain is of great importance. Collaboration occurs regularly with primary care, the regional health authority, benefits officers, family support, and other relevant parties. You contribute specialist competence in team meetings and participate in the development of Coordinated Individual Plans (SIP). An important part of the role is also actively participating in development work through environmental awareness, identification of improvement areas, and development of proposals. You also participate in professional networks to ensure that the organization maintains a high and current level of knowledge. Qualifications We are looking for you who are a licensed nurse with specialist education in dementia, elderly care, or other equivalent education. You have several years of experience working in the dementia field and a clear, documented commitment to the target group. You possess good clinical experience and are familiar with working with the BPSD registry. You have experience mentoring and training staff and feel comfortable in an advisory and supportive role toward both staff and collaboration partners. A B-category driving license is required for this position. Meritorious: Certified BPSD trainer and knowledge and experience with the Life Care management system. We place great emphasis on your personal qualities. To succeed in the role, we see that you are confident and pedagogical, with the ability to guide others, you create good and trustworthy relationships, and have good ability to collaborate in a team where you contribute specialist knowledge. Furthermore, you act in a structured and independent manner with a professional and person-centered approach. You see the whole picture around the person with cognitive illness and can coordinate measures based on this. You also engage in improvement and quality work. Other Trade union contacts: www.trelleborg.se/facket Please note that we have removed the personal letter from the application process and replaced it with qualification and selection questions. We are now seeking a personal assistant for a 65-year-old man with a muscular disease who lives in his own apartment. He uses a wheelchair and needs help with most aspects of his daily life. Since his disability is only physical, the client appreciates conversation, humor, and good cooperation in everyday life. As a personal assistant, you will be an important support in his daily life. Your tasks will include helping with personal care, transfers, meals, household chores, and other practical daily activities. Equally important is your ability to contribute to a safe, calm, and pleasant atmosphere where you work together to make daily life run smoothly. For the client, it is very important that you are kind, reliable, and attentive. You also need to have a great understanding that it is the client himself who decides in his home and in his daily life. He will guide you and explain how he wants different tasks to be performed so that it feels safe and comfortable for him. The foundation of the work is respect, clear communication, and good cooperation. The client's main goal is to create a safe and positive work environment where everyone feels comfortable and enjoys each other's company. You will also become part of an assistant group with extensive experience in personal assistance. According to the client himself, it's the best group he's ever had – and now you have the opportunity to become part of it. The work spans all hours of the day and night. Night shifts involve being on call sleeping at the client's residence. This is meaningful work where you truly make a difference in another person's life – while also having the opportunity to work closely with a person who has humor, life experience, and an open mind. Submit your application and tell us what you can specifically contribute – we look forward to hearing from you!
