About the Workplace Stuvsta Health Center is part of Praktikertjänst and operates on behalf of Region Stockholm. We currently have around 70 employees and over 22,000 registered patients. The health center is centrally located in Stuvsta centrum, close to commuter trains and buses. Job Responsibilities We currently have 12 staff members in home healthcare, including four nurses and one home healthcare physician. As a nurse in home healthcare, you are responsible for patient care planning, monitoring patient well-being, and medication management. You have close coordination with nursing assistants and physicians regarding the patient's well-being. As a nurse, you work from a holistic perspective with the patient's health in focus, and the goal is to provide the patient with personalized, safe, and secure care at home. We work in teams with other professionals such as district nurses, physicians, and nursing assistants, and we conduct rounds together with dietitians, physiotherapists, and occupational therapists. Your Profile We are looking for a nurse or district nurse for a substitute position starting in August for one year, with possible opportunity for limited extension. As a person, you are engaged, empathetic, self-directed, and have strong collaboration skills. You are flexible and take responsibility for both your work and the operation, and you have the ability to work in a structured manner during periods of higher workload. It is an asset if you have experience with basic home healthcare and knowledge of the TakeCare medical record system. A driver's license is, however, a requirement.
