Home Care Services Central District operates from Ekbacka in central Borgholm and serves care recipients from Borgholm down to the municipal border. Within our work group, we are divided into smaller teams where we work from a person-centered and rehabilitative approach, with a focus on continuity and quality in our work to provide good care and support. We are in an exciting development phase where we are developing our services through welfare technology, where we have now implemented medication dispensers which gives you the opportunity to be part of the future of home care. WORK TASKS Working in care and support means that through your work you truly make a difference in other people's daily lives. As a nursing assistant/care aide in home care, you have varied work tasks and work from a rehabilitative approach where the care recipient's needs and life situation affect how work tasks are carried out. Based on individual assistance decisions, you shall support people who have been granted home care services to manage their daily lives. Care recipients may need help with, for example, personal care, social activities, service tasks, and medication management. Your work tasks also include being a designated care contact, establishing and following up care plans and documenting in the Lifecare journal system. Collaboration in interprofessional teams is a large part of your work as well as independent work where you take your own responsibility. We use digital work tools where we, among other things, register visits, sign off on medications and document. You work in accordance with the Social Services Act (SoL), but also perform Healthcare services (HSL tasks) following delegation from licensed personnel. Work hours are distributed across day, evening and weekend shifts and you have the opportunity to submit your own schedule proposal within the work group within given parameters. Before an employment offer, a driver's license, valid identification in the form of a passport, and an extract from the police criminal record ("Extract for work in the home with elderly and people with disabilities") must be presented. This is done as a final step in the recruitment process. QUALIFICATIONS We are looking for you who: • are trained as a nursing assistant or care aide. • have a B-class driver's license • have very good knowledge of Swedish, both spoken and written • have a genuine interest in working with people and can meet each individual based on their individual needs and wishes • are positive, flexible, have a respectful and professional manner and good collaboration skills. • work from a rehabilitative approach • take responsibility for your work tasks and take own initiatives as well as show commitment • have the necessary computer skills, preferably in Lifecare Also advantageous: • experience with Senior Alert, BPSD • experience working in Lifecare Great weight is placed on personal suitability.
