About the Role As a nurse with us, you support the operation as needed and work closely with other professionals on the team. Your responsibilities may include: Counseling and patient contact Team visits (on-site and in homes) Medication management Assessment and triage of incoming cases Follow-up Patient and family support Documentation and administrative tasks related to patient work We work evidence-based and use digital tools to support follow-up and treatment.
