To create better conditions for collaboration, we have established a new joint organization for primary care and ambulance services: Close Care. The development means that we can more easily work together in multidisciplinary teams with changed working methods that better meet people's needs and conditions. There is a strong focus on developing the transition to close care together with other health centers, private care providers, and municipal operations. We have a shared vision and mission to provide the county's residents and visitors with better health and care. The health center with approximately 5,000 listed patients has fresh and appropriate facilities. Here we offer both acute and scheduled appointments. Adjacent to the health center is Närvårdsavdelningen (Näva), which we operate together with the municipality. DUTIES For this unique position, we are looking for you as a specialist in general medicine, where experience from work in home care and sheltered housing is a merit. Närvårdsavdelningen, Näva, in Strömsund is located adjacent to Strömsunds Health Center. Näva is a care unit in a rural area with patients in both inpatient care and municipal short-term care, which gives you a good perspective on the entire care chain. Here nurses and nursing assistants work together around the clock, and there are also rehabilitation staff, physicians, unit managers, cleaners, and janitors. Näva is a small work group that works closely together at a significantly lower pace compared to traditional hospital care. At Näva we care for patients where the majority are elderly and often have multiple illnesses with varying needs for nursing and medical interventions. We care for patients within all specialties except pediatrics. Common diagnoses include heart failure, cancer, various types of fractures, and infectious conditions. In both care settings, persons with need for palliative care in different phases are cared for. Palliative care is Näva's specialty where collaboration with the Palliative Care Unit in Östersund and Storsjögläntan exists. Duties at Näva • Rounds Monday-Friday • Patient admissions and discharges • Care planning The position comprises approximately 40% work at Näva, where there are great opportunities for developing the operations. For example, continued development of Good & Close Care, Digitalization, and more, medical leadership responsibility, etc. We are open and flexible for testing and implementing new working methods. The remaining 60% of the position is at Strömsunds Health Center. Here there is an opportunity to tailor a position with duties that suit you. This could, for example, be 20% child health care, 10% maternity health care, 20-60% work as responsible physician for sheltered housing, regular clinic work at the health center with your own patient list, example: if you work 60% at the clinic, it corresponds to 660 patients. Remote work opportunities are available. QUALIFICATIONS We are looking for you with specialist competence in general medicine with Swedish medical license and good ability to communicate in Swedish in speech and writing. You have Swedish language skills corresponding to at least level C1 according to the Council of Europe's scale. You enjoy working in teams with other professions, close to patients, and want to contribute to the development of the operation. As a person, we see that you are positive, humble, have good initiative, and prefer to work solution-oriented. We also appreciate that you have a structured, flexible way of working and are open to changes within the field of operations. Furthermore, you appreciate and understand the value of collaboration, but also take responsibility and work independently when required. Great weight will be given to personal suitability. For the position, a driver's license category B is a requirement. Employment form: Permanent employment. Primary care is a healthcare unit within the Region's Health and Medical Care Administration where higher education is conducted for medical degrees and research. This means that all applicants undergo expert review by the research and development responsible person prior to possible employment. Due to this, the application must contain the following documents: - A structured CV - Certificate of medical degree from the institution(s) where the physician has completed basic education. This also applies to physicians educated in another EU country and outside the EU. - Medical license from the National Board of Health and Welfare. - Specialist competence certificate from the National Board of Health and Welfare. - Documented supervisor training - If applicable, certificate of academic degree, for example, Doctor of Medicine or Docent, as well as date of defense and title of thesis. - If applicable, a complete updated publication list. - Certificate/documentation of teaching experience and any other pedagogical merits. - Any additional documented qualifications regarding, for example, clinical knowledge/experience, clinical development work, management experience, and anything else that you want to highlight as merits for the position sought.
