Physician Collaboration is now seeking nurses with placement in Varberg or Halmstad for key roles focused on coordinating, organizing, and following up on patients in close collaboration with our physicians, Halland Hospital, and municipal health and care services. Apply for the position here About the workplace Physician Collaboration is a region-wide operation that provides medical support to municipal health and care services. The mission currently includes physician support to short-term care facilities and is now expanding to also include vulnerable patients following hospital stays. The new mission involves medical follow-up and support over a limited period for patients at high risk of deterioration or readmission, regardless of living situation. We are now building a working model where the nurse's role becomes central to creating structure, continuity, and good collaboration around patients. About the position As a nurse in Physician Collaboration, you primarily work as a coordinator and organizer. The role makes you an important link between the hospital, physicians within Physician Collaboration, municipal health and care services, patients, and family members. Work is largely conducted via telephone and digital systems, with a focus on creating overview, coordinating interventions, and ensuring patients receive proper follow-up at the right time. Job responsibilities The role includes receiving and coordinating incoming cases and early creating a comprehensive picture of the patient's needs and situation. You maintain close contact with patients, family members, and care staff, where you follow up, check in, and contribute to security and continuity after hospital stays. By coordinating interventions between different stakeholders, you ensure the patient receives the right support at the right time. The work also involves planning and monitoring follow-ups, preparing cases together with physicians, and contributing to clear documentation. You are also involved in developing working methods and routines that strengthen quality and patient-centered care. Qualifications We are looking for you as a Specialist Nurse or Licensed Nurse with good clinical understanding and ability to work in a structured coordinating role. You have good judgment, can prioritize and create overview even when multiple cases are ongoing simultaneously. You are communicative and good at collaborating with different stakeholders, while working independently and taking responsibility for the whole patient picture. Experience from municipal health and care services, primary care, or working with vulnerable patients with complex care needs is meritorious. You are comfortable working via telephone and digital solutions and have an interest in developing working methods within close-to-home care.
