We are looking for you who want to work in municipal primary healthcare with a focus on residential care. Description We are looking for a nurse for the Municipal Primary Healthcare unit to work within the residential care team. You will be part of a team with nurses, but also in a multidisciplinary team with occupational therapist, physiotherapist, unit manager, support staff and other professionals surrounding the patient with a focus on patient-safe and person-centered care. You have great opportunity to plan and structure your workday in a way that works for you, your patients and colleagues. You will be the responsible nurse for one of our centrally located residential facilities in Sunne. Our residential facilities are centrally located within walking and cycling distance, but otherwise travel by car is included. You work mostly daytime on weekdays and approximately every 5th weekend and have flexible working hours. We offer individually adapted introduction, mentor and collegial supervision. We have varied, stimulating work with an open work climate where we support and help each other with challenging tasks. Work Tasks Your work tasks include coordinating and being responsible for ensuring that specific care interventions are planned, implemented and evaluated, as well as educating, supervising and delegating support staff. You perform medical assessments, specific care interventions and work on health-promoting and preventive work. The work involves you independently or together with colleagues planning and following up care interventions. Business systems that support us in our work include, among others, digital signature lists and the Life Care HSL journal system. Qualifications We are looking for you who are a licensed nurse with commitment and interest in municipal primary healthcare with a focus on elderly and dementia. It is meritorious if you have worked with similar tasks previously. You are welcome to have specialist training with a focus on district nurse, elderly care or psychiatry. To succeed in the position, you work independently and take great personal responsibility in your profession as well as for planning and follow-up of healthcare. You value teamwork highly and have a strong interest in working with frail elderly, dementia and palliative care. As a person you are stable and flexible, have a high sense of responsibility, and have the ability to work both independently and in a team. You are driven by challenges and want to develop both within your profession and within municipal primary healthcare. We place great emphasis on personal suitability with the ability to work with different specializations and in different groups based on operational needs. Conditions Swedish license as a nurse and several years of experience working as a nurse B-class driver's license (manual transmission) is a requirement and you must be able to cycle. Two reference persons, at least one supervisor, are provided at interview. In connection with possible employment, we request an extract from the Police's records and suspicion register.
