Description We're looking for a nurse for the Municipal Primary Care unit to work within the residential care team. You'll be part of a team with nurses, as well as in an interdisciplinary team with an occupational therapist, physiotherapist, unit manager, care staff, and other professionals surrounding the patient with a focus on patient-safe and person-centered care. You'll have great opportunities to plan and organize your workday so it works for you, your patients, and your colleagues. You'll be the responsible nurse for one of our centrally located residential facilities in Sunne. Our facilities are centrally located within walking and cycling distance, but otherwise travel by car is required. You'll work mostly daytime on weekdays and approximately every fifth weekend with flexible working hours. We offer individually tailored introduction, mentor, and peer supervision. We have varied, stimulating work with an open work environment where we support and help each other with challenging tasks. Responsibilities Your tasks include coordinating and ensuring that specific care interventions are planned, implemented, and evaluated, as well as educating, supervising, and delegating to care staff. You perform medical assessments, specific care interventions, and work on health promotion and prevention. The work involves planning and following up on care interventions independently or together with colleagues. The systems supporting us include digital signature lists and the Life Care HSL patient record system. Qualifications We're looking for a licensed nurse with commitment and interest in municipal primary care with a focus on elderly and dementia. It's advantageous if you've worked with similar tasks previously. You're welcome to have specialist training in district nursing, elderly care, or psychiatry. To succeed in this role, you work independently and take great personal responsibility for your profession and for planning and following up on healthcare. You value teamwork highly and have a strong interest in working with frail elderly, dementia, and palliative care. As a person, you're stable and flexible, have high sense of responsibility, and can work both independently and in teams. You're driven by challenges and want to develop both within your profession and in municipal primary care. We place great importance on personal suitability with the ability to work in different areas and with different groups based on operational needs. Requirements Swedish license as a nurse and several years of experience working as a nurse B driver's license (manual transmission) is required and you must be able to cycle. Two references, at least one from a supervisor, provided at interview. In connection with potential employment, we request a criminal record check from the Police.
