JOB DUTIES Are you a confident and dedicated nurse who wants to work with a holistic perspective and quality care? We are now seeking a nursing care coordinator who can contribute to patient safety and high-quality care for patients in home healthcare in ordinary residential settings. Within Landskrona Omsorg AB, there is an HSL team with expanded delegation responsibilities handled by home healthcare nurses. We are co-located at Kungsgatan 15 together with several professions, which strengthens collaboration and quality for patients in home healthcare, LSS, and care and residential housing. The assignment includes traditional duties for a nursing care coordinator in municipal primary care, such as: • Assess, plan, implement, and follow up on care interventions for patients with complex needs in municipal primary care. • Collaborate with internal teams and external partners (hospital care, primary care, ambulance) to ensure a cohesive care process. • Provide guidance, support, and encourage care staff, family members, and other caregivers to promote quality and safety in care. • Ensure correct documentation and reporting in accordance with laws, guidelines, and patient safety requirements. • Be in contact with medical technology and with patients who have complex care needs.
