Job Responsibilities As a case manager with us, you will be a key person in creating the conditions for a functioning daily life for people with disabilities and/or mental health issues. Your focus is on assessing needs, making decisions, and following up on interventions. Your main responsibilities are: Assessment: Gather relevant information to map the individual's needs. Evaluation: Analyze collected information based on applicable legislation, the municipality's established guidelines and procedures. Decision-Making: Write legally sound decision documents and communicate these to the individual. Coordination: Maintain close dialogue with relatives, representatives, service providers, healthcare services, and other authorities. Qualifications We are looking for someone who is confident in your professional role and enjoys working in an environment where no two days are the same. You have strong analytical abilities and can express yourself well verbally and in writing, as documentation is a significant part of your work. Education: Social work degree, social care program, or other relevant education. Knowledge: Good knowledge of LSS (Act on Support and Services for Persons with Certain Disabilities), SoL (Social Services Act), and FL (Administrative Procedure Act). Experience: Previous experience with authority management is a merit. Language: Excellent Swedish skills, both spoken and written. We place great emphasis on your personal suitability.
