What We Offer You We offer you a developing and varied combination position within home health services and the community care team in Söderköping. The role suits you if you are experienced in your professional role and driven by working close to the patient with a holistic perspective, where you can both work independently and as part of a team. The community care team is part of the initiative for Elderly Health and Well-being, where the focus is on creating a more cohesive, proactive, and person-centered care for our most vulnerable elderly. Work in the team is characterized by development, innovative thinking, and close collaboration between different professions and care providers. In combination with home health services, you have a varied daily routine where you meet patients in their home environment and are responsible for planning, implementing, and following up on care interventions. Nursing services within our LSS activities are also included in home health services. The work sometimes involves a high pace and complex situations, which places demands on prioritization ability, flexibility, and confidence in your professional role. What is specific about this position is the alternation between independent work in home health services and the team-based, more coordinating work in the community care team. You work closely with colleagues in both municipal health and social care, care staff, rehabilitation staff, as well as doctors and other external care providers. Work in home health services includes: Assessment, planning, and follow-up of health and care interventions in the patient's home Medication management, medical procedures, and medication delegation Collaboration with care staff and other care providers Documentation in accordance with applicable legislation Work in the community care team involves: Proactive and person-centered work focusing on vulnerable elderly Coordination of interventions around the individual In-depth medical assessments Collaboration across organizational boundaries Participation in development and improvement work The position involves significant individual responsibility and requires good organization, ability to prioritize, and the ability to handle complex care situations. The position includes weekend work, which currently is every sixth weekend. Qualifications Requirements: Licensed nurse or district nurse Several years of experience in municipal health and care services Good knowledge of Swedish, both spoken and written Driver's license category B Meritorious: Experience working with elderly and/or multimorbid patients Experience of collaboration between different care providers Experience of improvement or development work Experience with the Lifecare management system Personal Competencies: You are quality-conscious and work in a structured manner with focus on patient safety. You have good judgment and can make well-founded decisions in complex situations. You are flexible and able to adapt to changing needs in the operation. The role requires a high degree of independence combined with excellent collaboration skills and a good ability to prioritize during high workload. We place great emphasis on personal suitability.
