At Bromma Health Center's home care operations, we are a team of 3 nursing assistants, one full-time district nurse, and a home care physician at 20%. We are a dedicated and highly competent team of doctors and nurses who together run a small, personal health center in the middle of Brommaplan. Our goal is to offer care of the highest quality with support from the latest tools and methods within modern health and medical care. About Us At Bromma Health Center, we conduct a primary care practice on behalf of Region Stockholm. We have a growing home care operation, active chronic disease clinics, and an attractive first-line service for adults. As a smaller health center with close collaboration between professions, we create good continuity and high quality care. Now we are growing – and looking for more dedicated colleagues to join our team! Work Tasks / Responsibilities What does the role of district nurse in home care involve? That you: are responsible for quality control visits to our home care patients. act as a coordinator for the operation's three nursing assistants and are a first point of contact for clinical questions about patients. perform daily monitoring of LifeCare and our direct phone line for patients 75 years and older. act as a liaison between the home care operations and other stakeholders such as patients' close relatives, legal guardians, external case managers, etc. work closely with the home care physician and management for clinical assessments and planning of the home care operations. Daily Work Tasks Daily rounds with home care nursing assistants. Visits and spot checks of the health center's home care patients, possible SIP visits. Monitoring of LifeCare. Staffing our direct phone line for patients 75 years and older, 1 hour per day. Main Goals and Challenges of the Role Provide safe, continuous, and evidence-based care. Gather and structure sometimes complex care needs for patient-safe care. Create continuity in home care quality. Coordinate care for patients with complex needs. Contribute to the operations' development and good quality. Qualifications / Requirements Completed district nursing education with previous experience in basic home care. We are looking for you with good organizational skills in your work, someone who brings colleagues together and drives the group forward in a goal-oriented manner with good collaboration and communication skills. You are empathetic and attentive in patient interactions, organized and responsible with good judgment, initiative-taking and development-oriented, and a team player who works with a positive attitude without pretense.
