Your tasks • Reception work with assessments, counseling, and triage. • Telephone advice and digital patient contact. • Wound care, injections, specimen collection, and other practical procedures. • Health conversations and patient education. • Follow-up of chronic diseases (e.g., diabetes, hypertension, COPD) depending on competence. • Collaboration with other care providers and authorities. • Documentation and quality work. You work in a team with a colleague and have close collaboration with the physicians responsible for the patients in your team. In our telephone advice service, you as a nurse call patients in your team in the first instance. This way you have the opportunity to get to know your patients over time and make a real difference.
