Open Care and Outreach Description We are looking for an engaged and professional Open Care Clinician who wants to contribute to improving the quality of life for residents in Perstorp. The new Social Services Act emphasizes early, preventive, and easily accessible services for children, youth, and families. In our transition work, we aim to meet people where they are and offer support before problems escalate. We aim to provide the right treatment and support at the right time for each individual. Within our unit, we work with both non-assessed interventions and needs-assessed assistance, with a strong focus on preventive and early interventions to reach residents in the municipality who need support at an early stage. Services are provided both individually and in groups; the unit also provides various types of meeting places. We are now looking for engaged clinicians who want to work with intensive treatment in the home and help strengthen families' own resources in their everyday environment. Work Tasks In your role as an Open Care Clinician, you will play a central role in supporting and strengthening residents in Perstorp. You will work with: Conducting individual assessments, creating personal treatment plans, and providing qualified treatment interventions in families' home environments based on each individual's needs and circumstances. Working on change processes together with children, youth, and guardians both individually and in groups, and strengthening families' ability to manage everyday challenges. Leading and participating in discussion groups and activities aimed at promoting mental health and quality of life. Collaborating with schools, health and medical services, police, and other important stakeholders to ensure coordinated care for individuals. Working preventively to reduce the need for more intrusive interventions. Documenting and following up on interventions in accordance with applicable legislation and business guidelines. Qualifications We are looking for someone with a degree in social work, social pedagogy, or equivalent higher education that the employer deems appropriate. You have experience working with children, youth, and families in a treatment context. You are comfortable working independently while valuing collaboration. You have the ability to build trustful relationships even in complex situations. You have good ability to document interventions in accordance with applicable legislation. It is an advantage if you have training in, for example, DBT, CBT, MI, or other evidence-based treatment methods.
