The Mobile Healthcare Team provides care in patients' homes for those who have difficulty visiting their regular healthcare provider. The team is organizationally part of Primary Care Operations in Region Gävleborg. We work broadly and collaborate with all health centers in the area, both corporate-run and privately-run, as well as with municipal healthcare and departments at Gävle Hospital. In the team, physicians and nurses work together to ensure professional and safe care. The operation is based out of a team vehicle that travels throughout Gävle Monday through Friday to enable close and coordinated care services for our patients. WORK DUTIES The mobile healthcare team's work is based on a comprehensive assessment of the patient's situation in their home environment. Many of our patients are elderly with multiple conditions, and performing assessments and treatment in the home setting provides great added value for the patient. As healthcare providers, we encounter the whole person's life circumstances, which gives us invaluable guidance in our work. A large part of this work involves maintaining and developing collaboration with primary and specialist care as well as various municipal actors. Our work also includes supporting people during their final time in life. You are confident in your professional role and have an empathetic and professional approach. You are flexible in your working methods and adapt easily to changing situations and operational needs. At the same time, you work in a structured manner and have a strong ability to plan, prioritize, and organize your work efficiently. You are proactive and gladly contribute with solutions that create added value for patients. You see opportunities in change and actively contribute to developing working methods and improvements that strengthen care and operations. In your role as a specialist physician, you must: • be a licensed specialist physician with authorization to work in Swedish healthcare • hold a valid B driving license • have excellent knowledge of Swedish, both spoken and written It is advantageous if you have specialist competence in general medicine or another relevant specialty, such as geriatrics, internal medicine, cardiology, or infectious medicine.
