Physician Support is now recruiting nurses with placement in Varberg or Halmstad for key roles focusing on coordination, management, and follow-up of patients in close collaboration with our physicians, Halland Hospital, and municipal health and care services. About the Workplace Physician Support is a region-spanning operation that provides medical support to municipal health and care services. The assignment currently includes physician support to short-term care facilities and is now expanding to also include vulnerable patients following hospital stays. The new assignment involves medical follow-up and support for a limited period for patients at high risk of deterioration or readmission, regardless of living arrangements. We are now establishing a working method where the nurse's role becomes central to creating structure, continuity, and good collaboration around patients. About the Position As a nurse in Physician Support, you will work primarily as a coordinator and manager. The role means you are an important link between the hospital, physicians within Physician Support, municipal health and care services, patients, and their families. The work is conducted largely through phone and digital systems, with a focus on creating overview, coordinating measures, and ensuring that patients receive proper follow-up at the right time. Responsibilities The role involves receiving and coordinating incoming cases and early on creating a comprehensive picture of the patient's needs and situation. You have close contact with patients, relatives, and care personnel, where you follow up, check in, and contribute to security and continuity following hospital stays. By coordinating measures between different stakeholders, you ensure that the patient receives the right support at the right time. The work also involves planning and monitoring follow-ups, preparing cases together with physicians, and contributing to clear documentation. You are simultaneously involved in developing working methods and routines that strengthen quality and patient-centered care. Qualifications We are looking for you who are a Specialist Nurse or a Licensed Nurse with good clinical understanding and the ability to work in a structured manner in a coordinating role. You have good judgment, can prioritize and create overview even when multiple cases are ongoing in parallel. You are communicative and find it easy to collaborate with different stakeholders, while working independently and taking responsibility for the overall picture of the patient's care. Experience from municipal health and care services, primary care, or work with vulnerable patients with complex care needs is meritorious. You are comfortable working through phone and digital solutions and have an interest in developing working methods within close care.
