Registered Nurse for the Planning Team, Home Care Administration

Your Tasks The Home Care Administration is seeking a registered nurse to join the Planning Team for Rehabilitation, Care and Health Services (PROV), which collaborates with Region Halland to ensure safe discharge of patients from Halland Hospital to home care. PROV is an interdisciplinary team consisting of social workers, registered nurses, occupational therapists, and physical therapists. The planning team is based at Skolgatan. PROV's mission is to facilitate and streamline the care flow from inpatient care to municipal home health services through clear care plans. Patients should experience a safe and secure transition to other forms of care and support services upon discharge from Halland Hospital. Care planning is conducted almost exclusively digitally via TEAMS. In case of lower workload on PROV, work assignments may be carried out at other units within the administration. In home healthcare, we currently employ over 100 registered nurses in our operations, which run 24/7. We take pride in our elderly care and can offer exciting and varied work. As a person, you are responsible and motivated to organize work with your colleagues in the best possible way. Experience from Halmstad Municipality's home healthcare is a merit. You should have the courage and willingness to work both independently and as part of the professional team and with other stakeholders. We believe you will thrive with us if you are challenged by situations requiring innovative and creative thinking. The position also requires good computer skills, as it includes documentation and planning in our systems and tools. A driver's license for a car with manual transmission is required for this position. Personal suitability will be given great weight.
