The Social Services Department in Askersund Municipality is responsible for elderly care, LSS and care services, social psychiatry, activities for unaccompanied refugee children, and individual and family care. The Health and Medical Care Unit also falls under the Social Services Department and is undergoing exciting development where we are investing in new roles to strengthen operational quality work for our patients. Therefore, we are also looking for nursing care responsible nurses for both home care and special housing! 2 position(s). JOB DUTIES Nursing/Nursing Science - Assess the needs and resources of the patient and next of kin from a salutogenic and holistic perspective. - Observe and act on changes based on changes in health status. - Support the patient's participation in care and self-care ability. - Reflect on and take action to treat, alleviate disease and symptoms in relation to the patient's life world. - Work with quality and operational development. - Conduct specific nursing care and symptom relief at the end of life. - Critically evaluate different treatment methods. - Apply a team-based approach to meet the patient's and family's needs and offer supportive and advisory conversations. - Work from a holistic perspective with patient/next of kin/team. - Utilize the multidisciplinary team's knowledge. - Take advantage of the patient's own resources for the best experience of well-being. - Responsible for medical-technical aids used in home care. Treatment, Information and Education - Use pedagogical techniques to strengthen the patient's decision-making. - Initiate and conduct difficult conversations with patients and their next of kin. - Guide, teach and evaluate students based on the profession. - Guide nursing work and educate care personnel. - Respect autonomy, integrity and dignity of patient/next of kin. - Participate in ethical discussions and decisions. - Implement and arrange training regarding delegation according to current regulations and guidelines. - Conduct own development work and keep up to date with new research findings. Security & Quality - Work independently based on relevant national guidelines, local care programs and national quality registers. - Identify factors that pose a threat to safe care and work systematically to prevent care injuries. - Handle patient information in such a way that patient safety and confidentiality are always guaranteed despite continuity breaks due to various laws and operations. - Work according to applicable laws, regulations and guidelines and standards for home care. - Create an appropriate work and care environment. - Identify needs and initiate contact with physiotherapist and occupational therapist to ensure that the patient receives the right aids and home adaptation. - Initiate, lead and participate in quality development. QUALIFICATIONS We are looking for you who · Are a licensed nurse or specialist nurse. · Have experience in municipal health and medical care. · Have a good understanding of laws and regulations within HSL. · Preferably have experience in quality development and improvement work. Personal Qualities · Development-driven and solution-focused. · Flexible, structured and collaboration-oriented. The position requires a Class B driver's license. Salary form: Monthly salary.
