Description Communal Rehab is organized within the Social Services Department in Rättviks Municipality. The unit also includes an activity that supports patients recently discharged from hospital according to the SUS law. The unit also has housing coordination, a dietitian, and a short-term ward. The unit's main mission is to contribute to the municipality's goal of making residents as independent and active as possible by motivating and supporting patients to maintain and develop their active abilities. The target group is young adults, adults, and elderly in ordinary housing, group living arrangements (SÄBO), and supported housing under the LSS. The work is characterized by commitment, innovation, development, and customer focus. Currently, rehab consists of 4 occupational therapists and 5 physiotherapists. As one of our physiotherapists has chosen to pursue further education to meet the rising age curve, we are now seeking a replacement to start as desired. Job Description As a physiotherapist, you will be responsible for assessment, rehabilitation, prescriptions, delegation to nursing staff, and assistive device prescription. We value interdisciplinary teamwork so that our patients feel secure and involved in their daily lives and to create opportunities for them to remain at home. In Rättviks Municipality, we work with Safe Discharge after hospitalization, which involves quick and more intensive interventions together with a nursing assistant assigned to Rehab and home care services in connection with the patient's return home from the hospital. Your main responsibilities as a physiotherapist are to perform transfer assessments and assessments of physical prerequisites for training. You will design individual exercise programs and instruct/train nursing staff in daily rehabilitation and transfer techniques. Prescribing assistive devices is part of the position, as is participating in the municipality's work on fall prevention, preventive and development initiatives. We document in Lifecare HSL. Qualifications We are looking for someone who is a licensed physiotherapist. It is meritorious if you have good knowledge of regional and municipal healthcare services, as well as previous experience with multiprofessional teamwork. We welcome an interest in environmental awareness and development within the healthcare and care sector. Since communication and documentation are part of the work tasks, we want you to have digital competence and master the Swedish language in speech and writing. As the work involves home visits throughout our vast municipality, you need to have a valid driver's license for a personal car. As a person, you have good collaboration skills, you can motivate and inspire those around you with your rehabilitative/habilitative approach. You enjoy working both independently and as part of a team.
