The Mobile Healthcare Team provides care in patients' homes for those who have difficulty visiting their regular healthcare provider. The team is organizationally part of VO Primary Care in Region Gävleborg. We work broadly and collaborate with all health centers in the area, regardless of management, as well as with municipal healthcare and departments at Gävle Hospital. Doctors and nurses work together in the team to ensure professional and safe care. The operations are based out of a mobile unit that runs Monday-Friday throughout Gävle to enable close and coordinated care services for our patients. JOB RESPONSIBILITIES The mobile healthcare team's work is based on a comprehensive assessment of the patient's situation in their home environment. Through team assessment and coordination of resources, we ensure that patients receive the right care at the right time. The work includes contact with patients, family members, and other healthcare providers to plan home visits and manage incoming patient cases. In the role as district nurse, you should • be a licensed district nurse • have very good knowledge of Swedish, both spoken and written • hold a valid B-class driver's license.
